Accounts Assistance

Elite will submit claims to all third parties such as Medicare, Medicaid, Commercial Insurance, Car Insurance, Worker’s Compensation, etc. If you receive an invoice and would like us to submit a claim, contact our team and provide us with the insurance information or email the details to:

One of the hallmarks of this commitment is the provision of high-quality services in full compliance with all laws, codes, ordinances, regulations, and contractual obligations.

Code of Conduct

Elite has complete confidence in the integrity and ethical conduct of its staff members. To fortify existing conduct, our Code of Conduct assists all staff members in avoiding both the appearance and commission of improper activities. The Code of Conduct is a component of our total Compliance Plan which is available upon request.

Notice of Privacy Practices

Elite's commitment to compliance and to making a positive impact includes maintaining all Protected Health Information (PHI) in accordance with all relevant federal and state law. The following Notice of Privacy Practices, in English and Spanish describes our duties, responsibilities and your legal rights. If you need to obtain a copy of your Medical Records please contact our Patient Financial Services department and they will assist you.

Compliance Reporting

If any patient, employee, vendor, or contractor has a question regarding Elite's potential noncompliance with our Code of Conduct, they can call the submit an anonymous notice at Elite's Website, www.elitemedical.org. The contact form at the bottom of the page does not require any identification but if you would like us to provide you with feedback, you can include this information. You can also call our Compliance Officer directly at (915) 351-3224 or email at complianceofficer@elitemedical.org.
